The first time you visit our office, we want the experience to be one of welcome as well as efficiency. We have provided all of the New Patient Forms that you need to complete under the New Patient Information section of the site. Please plan to arrive about 20 minutes before your scheduled appointment time to ensure that you have all information prepared and can be introduced to our office.

At Young & Associates, we appreciate confirmation of your appointment time and you can expect a call from our staff as a friendly reminder. We certainly understand changes in appointments and appreciate a 24 hour notice.
